I would also like to add, that if you do the steps to Unmount:

1. Unmount Lun

2. Detach Device

3. Have your SAN then revoke access to your hosts

4. re-scan your HBAs

there is a step #5 that I only see mentioned in 1 KB

5. do an esxcli storage core device detached remove -d

This will remove the device you detached that the hosts still know about.
