I'm having a very similar issue at present with SAS attached IBM storage on IBM hosts.
Everything works as expected, until a host is shutdown or rebooted - after this, all VM's become inaccessible and the VMFS volumes disappear. Storage rescan or similar do not help. This has happened without fail every time a host is rebooted, I've found the only solution thus far is to reset the host configuration and re-configure the host.
I'm seeing almost identical log output also:

I've seen a number of posts with similar symptoms but these are almost always iSCSI issues that are resolved by correcting the iSCSI configuration, I've seen little or nothing that addresses these issues when using FC/SAS storage.
Any suggestions would be appreciated greatly!
