I haven't used NPIV myself, but as far as I understand, your VM merely sees the FC LUNs you present as ordinary SCSI disk devices on it's virtual SCSI controller, just like it would with a normal VMDK...

Definitely option 2, as it scales better and is easier to manage on both ends. Option 1 could have some use, if you are replicating LUNs or using SAN snapshots, but I would say it is the exception.
